Our Personalized Day Supports offer a more individualized alternative to traditional day programs, providing one-on-one support built around each person’s interests, goals and preferred activities.
Heart to Heart’s 24-Hour Support Model provides continuous, around-the-clock support for individuals who benefit from a higher level of assistance while continuing to live in a comfortable, familiar home environment. With dedicated staff available at all times, individuals receive personalized support that promotes safety, stability, independence and overall well-being.
Self-directed services offer families greater control and flexibility in caring for a loved one. When you partner with Heart to Heart, you get more than just payroll support — you gain access to agency-exclusive benefits not available through a fiscal intermediary like Acumen Fiscal Agent (formerly PPL).
Families who self-direct with Heart to Heart benefit from staff perks like health insurance and paid time off. We also offer emergency on-call support, activity planning, staff training in care and crisis response, as well as respite, behavioral supports, and skilled nursing. We support individuals with grocery shopping, ensuring they have the necessary items for their dietary needs. This includes making shopping lists, navigating stores, and managing purchases.
We prioritize the health and safety of our clients by offering continuous supervision and assistance. Our staff are trained to handle emergencies and provide support for health-related needs.
Our staff assist with housekeeping tasks such as cleaning, organizing, and maintaining a tidy living environment. This helps individuals live in a comfortable and hygienic home.
We coordinate and provide transportation to medical appointments. We assist individuals during their visits, ensuring they understand medical advice, follow through with prescribed treatments and maintain documentation.
Our DSPs help individuals manage their medications, ensuring they take the correct doses at the right times. We provide oversight to ensure safe and effective self-medication practices and self-medication training and assessments.
We help individuals manage their finances, including budgeting, banking, and bill payment. Our goal is to promote financial independence and ensure individuals can effectively handle their finances. We also assist in applying for Representative Payee for Social Security Benefits.
We work with individuals to develop and enhance their problem-solving skills. This includes addressing daily challenges, making decisions, and finding appropriate solutions.
Our DSPs assist with planning and preparing nutritious meals that meet dietary requirements and preferences. We emphasize the importance of balanced diets and healthy eating habits to maintain overall well-being.
Safe and reliable transportation services are provided for various needs, including medical appointments, social activities, shopping, and other community engagements.

Heart to Heart’s Individual Supports and Community-Based Supports are designed for adults with intellectual and developmental disabilities who are eligible for services through the New Jersey Division of Developmental Disabilities (DDD).

The model Heart to Heart has created with all our programs, allow us to provide 1:1 support for as many hours as your loved one has been authorized to receive.
Our DSPs assist clients with scheduling, transportation, and attending medical appointments. We maintain open communication with healthcare providers and families to ensure coordinated care. Our staff will communicate with the family before and after every appointment to ensure that all concerns are addressed.
Our program offers one-on-one support for as many hours as you are authorized to receive. We tailor our services to meet the unique needs of each individual, ensuring they receive the care and assistance they need.
Yes! This is an excellent option for those who may struggle in group settings but still want to stay engaged and active in their community. Our Community Supports Manager will work with you to create a customized plan for community involvement that aligns with your individual supports and desires.
