Since we founded Heart to Heart (H2H) in 2007 in East Orange, NJ, we have grown into a leading provider of services for individuals with Intellectual/Developmental Disabilities (I/DD). Thanks to our commitment to excellence and personalized care, we have expanded beyond New Jersey to serve communities in Pennsylvania as well. Today, we operate 30 Residential Group Homes, Supervised Apartments, and comprehensive Day Programs. As a result, we have touched the lives of hundreds of individuals and their families, helping them achieve greater independence and fulfillment.

Our mission is to help individuals with I/DD maximize their independence and growth within the community by offering a variety of services tailored to meet the unique needs of each person. For example, we provide Individual Supports (in-home or community-based), Group Homes, Day Program Services, Pre-Vocational Support, and Behavior Supports.
We are committed to providing high-quality customized services that address the needs of individuals with developmental disabilities. Furthermore, our team adheres to the highest standards of professional practice and individual supports. We support our employees in enhancing their skills and knowledge through rigorous training exercises to maintain top-tier care.
As a qualified Home and Community-Based Supports (HCBS) provider, approved by NJ and PA Medicaid, Heart to Heart offers services to individuals with various medical and behavioral needs. Additionally, our accreditation through the Council of Quality and Leadership reflects a commitment to continuous improvement and quality support.
For over 13 years, we have supported hundreds of individuals and their families in the New Jersey and Philadelphia area. We are continuing to expand our services to meet the needs of individuals with acute medical and behavioral challenges, with additional Group Homes, Day Programs, and Behavioral services planned.
We have a state-wide management presence with regional offices throughout New Jersey. Moreover, our staff includes a full team of clinical, administrative, recruitment, and QA professionals. These regional offices serve as training centers for our Direct Support Professionals (DSPs), enabling us to effectively serve an increasing number of individuals.
