Direct Support Professionals (DSP) provide direct care and support to individuals with developmental disabilities in our residential group homes. Responsibilities include assisting residents with daily activities, implementing Individual Service Plans (ISPs), coordinating recreational activities, and administering medications. Staff members work closely with residents to foster a nurturing environment and promote community inclusion. A high school diploma, valid NJ driver’s license, and ability to pass a background check are required. Previous experience working with individuals with developmental disabilities is preferred.
Licensed Practical Nurses provide nursing care and support to individuals with developmental disabilities in our group homes. Responsibilities include administering medications, implementing care plans, assisting with daily activities, and coordinating medical appointments. LPNs work closely with the Group Home Manager and other staff to ensure residents receive comprehensive care tailored to their individual needs. Candidates must have a valid NJ LPN license, be able to pass a background check, and possess a NJ driver’s license with a good driving history. Experience with developmental disabilities is preferred.
Registered Nurses at Heart to Heart Home Care provide oversight and direct nursing services within our residential programs. Responsibilities include supervising nursing staff, developing care plans, conducting training, attending ISP and IDT meetings, and coordinating medical services. RNs play a key role in ensuring high-quality care by overseeing medication administration, attending appointments, and monitoring residents’ health needs. Candidates must have a valid NJ RN license, two years of nursing experience, and a NJ driver’s license. Experience working with individuals with developmental disabilities is preferred.
The Behavior Support Specialist provides behavioral support services to individuals with developmental disabilities in our Group Homes. This role involves assessing behavioral needs, developing and implementing intervention plans, and providing training to staff and individuals to promote independence and enhance quality of life. The specialist collaborates with support staff, families, and other professionals to ensure continuity of care and responds to crises as needed. A valid NJ driver’s license, clean criminal background check, and relevant credentials are required.
The Board Certified Behavior Analyst (BCBA) develops and implements Behavior Support Plans and skill programs, trains staff and parents, records and reports data, and collaborates with interdisciplinary team members. This role requires a Master’s degree in a related field, BCBA certification, and at least one year of experience working with individuals with intellectual/developmental disabilities. Preferred qualifications include crisis management training and experience conducting Functional Behavior Assessments and creating function-based plans. The BCBA observes patients, conducts assessments, modifies plans based on data, and provides support and training to caregivers. The BCBA also coordinates care, prepares reports, and ensures the safety and confidentiality of individuals receiving services.
The Registered Behavior Technician (RBT) provides direct 1:1 support to individuals with developmental disabilities by implementing Behavior Support Plans and skill acquisition programs under the supervision of a Board Certified Behavior Analyst (BCBA). This role involves teaching communication, daily living, and social skills, assisting with community integration, and collecting accurate data to track progress. RBTs collaborate with interdisciplinary team members, follow treatment plans with fidelity, and maintain a safe, supportive environment. This position requires an active RBT certification, a high school diploma or equivalent, and at least one year of experience working with individuals with intellectual/developmental disabilities. Preferred qualifications include crisis management training, strong communication skills, and the ability to adapt to the unique needs of each individual.
You must be 18 years or older, hold a Non-Probationary Driver’s License and HS diploma (or equivalent), in addition to a clean background check and drug test. Experience working with individuals with disabilities is preferred but not mandatory as we provide comprehensive training.
