When you sign up for In-Home Services, you don’t just get a direct support professional trained to assist your loved one. Of course, that’s part of it. But you also get the full weight of an agency that operates like a family: a dedicated Community Supports Manager, access to skilled professionals, thoughtful planning, and people who notice when something doesn’t seem right and step in to fix it.
It’s not a one person job. And we never treat it like one.
In most agencies, in-home support means a single person shows up and clocks in. But at Heart to Heart, there’s always someone else behind the scenes making sure things run the way they should.
Our Community Supports Managers are the thread that ties it all together. They help with staffing, visit regularly to check in on care, and offer real-time support if something ever feels off. They know your loved one by name, not by a file. And they are trained to catch the details that matter: changes in behavior, progress toward goals, small wins, or new needs.
They also make sure families aren’t left figuring things out on their own — from navigating IDT meetings and entitlements to managing medical records, critical incident reporting, or coordinating with behavioral and nursing supports.
For individuals who aren’t thriving in traditional facility-based settings, our My Day My Way program offers a fresh approach. Running from 9 AM to 3 PM, Monday through Friday, this model was created for those who want to live and learn at their own pace, choosing how they spend their time. That might mean learning to cook a favorite recipe, practicing money management, or simply going for a walk in the neighborhood park.
We’ve found that when people have control over their day, they feel more confident, more engaged, and more empowered. And when that happens, real growth begins.
Of course, support isn’t limited to the four walls of a home. Our Community-Based Supports are all about encouraging real-world experiences such as shopping, social outings, recreation, volunteering, and more. We help craft routines that reflect the individual’s interests and goals, not a one size fits all schedule.
