A Look Ahead: Our Vision for 2025 

As we approach 2025, the landscape of services for adults with developmental disabilities is shifting in meaningful ways. These changes bring new challenges, but also open doors to incredible opportunities. At Heart to Heart, we’re proud of how far we’ve come and excited about what’s next. Together, we’ll continue to grow, innovate, and make a difference.

Here’s a glimpse into our vision for the future and the initiatives that will guide us as we strive to empower the individuals we serve.

 
Expanding Access to Quality Services

Our top priority remains expanding access to safe, supportive, and high-quality residential services. We recognize the growing need for specialized housing that not only provides security but also fosters independence, community involvement, and personal growth.

To meet this need, we’re planning to open six additional group homes in New Jersey and two more in Pennsylvania by the end of 2025. These homes will be thoughtfully designed to ensure accessibility, comfort, and individualization—creating nurturing spaces where residents can thrive.

We’re also enhancing services within our existing homes. New support programs, including life skills training and vocational coaching, will help residents build independence, deepen social connections, and achieve personal goals.

 
Embracing Technology to Enhance Care

Technology is transforming the way we support individuals with developmental disabilities. Over the next few years, we’ll embrace innovative solutions to provide even better care and coordination.

One exciting initiative is the rollout of a digital care coordination platform. This tool will improve communication among staff, healthcare providers, and families, ensuring personalized care plans are seamlessly managed and monitored.

Additionally, we’re exploring assistive technologies to enhance independence. From communication devices to smart home systems, these tools can empower individuals to take greater control over their daily lives. We plan to pilot these technologies in several group homes by the end of 2025 and explore broader implementation based on their success.

Our mission is only possible because of our incredible team. That’s why one of our core goals is to invest in our staff, ensuring they feel supported, valued, and equipped to provide exceptional care.

In the coming years, we’ll expand training opportunities in key areas such as behavioral support, crisis intervention, and person-centered care. We’ll also focus on creating pathways for mentorship and career advancement while enhancing benefits to promote staff well-being.
